Facilitates the transfer of patients to/from other hospitals, and participates as the clinical team lead during interfacility neonatal critical care transport. B. Verifies That the Quality of Care Is Documented in the Medical Records 1. Elicits, records and interprets the past medical, psychosocial and obstetrical history of the neonate's family. 2. Documents total plan for delivery of care. Writes orders for accomplishing that plan and implements the plan personally or through collaboration with medical and nursing staffs. 3. Evaluates and revises plan of care on an ongoing basis, and reviews revisions with Neonatologist. 4. Assists with discharge planning/teaching. C. Participates in Scholarly Activities 1.  Disseminates nursing research findings and incorporates these findings into nursing practices for patient care within the NICU; participates in research endeavors and program development. 2.  Identifies educational needs of nursing staff regarding the care of patients in the NICU. This involves supervising medical assistants, ECG technicians, Holter monitor technician, nurses and clerical staff. Responsibilities include managing day-to-day operations in our busy clinical practice in Wilmington. This includes managing daily patient flow, training staff, supervising nurses in phone triage, documentation, performance of cardiac-related studies, sedations, coordinating preoperative surgery patients, facilitating admissions from clinic, and managing subspecialty groups of patients with the assistance of the patient’s cardiologist. Also responsible for managing the day-to-day activities in the ECG lab and Holter lab, ensuring that studies are performed, interpreted, resulted and billed in a timely fashion. Although this position primarily resides in our Wilmington office, this position will have management input into processes that span across our 14 satellite offices in the tri-state area (DE, NJ, PA). Responsibilities Provides registered nurse services in the Cardiac Center's Outpatient Clinic under the direction of the Chief of Cardiology and Director of Clinical Operations. Provides age- and developmentally appropriate care to infants, children and adolescents. Provides clinical assistance to cardiologists. Responds to and provides direction during crises and emergency situations. Assumes responsibility for the coordination and integration of interdepartmental and intradepartmental services. Provides leadership and direction to patient care team members in accordance with organizational goals. Responsible for the patient care delivered by other clinical staff assigned to the Cardiac Center’s Outpatient Clinics at AIDHC and cardiac-specific satellite offices (in NJ and DE), and provides advice and guidance to other health care providers. Maintains clinical skills to provide coverage to staff during times of short staffing. Reconciles ECG and other studies done to charges entered into Epic. Maintains accurate patient logs and databases. Makes sound decisions, implements strategies and evaluates outcomes. Provides management and leadership of other patient care units within the Cardiac Center when necessary. Acts as a change agent. Supports attendance at continuing education programs/seminars within budgetary constraints. Supervises coordination of equipment and supply purchase within the Outpatient Clinic. Negotiates equipment and supply pricing. Responsible for the economical utilization and maintenance of equipment and supplies. Ensures the continuous assessment and improvement of quality patient care. Ensures the safety of patients and employees by providing sufficient training on all equipment and procedures and maintaining clinical proficiency of all clinic staff. Participates in departmental and hospital programs for quality assessment and improvement, and works with department management to improve the services provided. Responsible for new and ongoing quality improvement projects and metrics-reports data; analyzes and plans for improvements. Membership required on Cardiac Care Committee. Assists in the budgeting process and cost accounting procedures for the Cardiac Center’s outpatient practice. Ensures patient-focused unit(s) deliver quality services within budgetary guidelines. Works with staff to coordinate care with health care providers of all disciplines to achieve patient care goals. Assesses and evaluates patient care needs, personnel needs and departmental needs to increase productivity of the unit. Provides leadership and direction for interviewing and selection of patient care team members. Prepares and conducts employee performance after probationary period and annually. Responsible for guidance and counseling of employee work performance. Plans Cardiac Center's Outpatient Clinic schedule to provide adequate coverage in support of clinic operations and physician needs. Enters clinic staffs' time into Kronos. Develops, supports and facilitates hospital direction and goals. Primary Function Registered Nurse with responsibilities for assisting with the professional role development of members of the Nursing Department, as well as the implementation of unit programs related to breastfeeding and the use of breast milk to improve the quality of care delivered. The Lactation Consultant (LC) is a certified Registered Nurse skilled in assessment, planning, implementation and evaluation to assist the breastfeeding mother-baby dyad with feeding and to help the mother to achieve her individual goals. The LC functions as a member of the multidisciplinary health care team as a consultant, educator and mentor to nurses, health care professionals, patients and their families. The LC demonstrates behaviors that model the core values and mission statement for the organization. The LC facilitates the Delaware Women Infants & Children (WIC) contract for breastfeeding peer councilors. The LC supervises the breastfeeding peer councilors who practice at the Nemours satellite offices. Essential Functions Clinical Judgment: Demonstrates clinical expertise while supporting the breastfeeding mother-baby dyad. Provides advanced assessment and interpretation, implements nursing care plans and support, and evaluates outcomes for the breastfeeding population based on physician/nurse referrals for all breastfeeding or pumping issues. Maintains professional practice knowledge base, including departmental and organizational policies, standards of care, and procedures. Participates in and uses evidence-based research to identify strategies to improve professional development activities, nursing practice and patient outcomes. Demonstrates clinical expertise through research, demonstration, observation and teaching of clinical skills. Facilitator of Learning: Provides evidence-based education to direct care nurses and families. Utilizes principles of adult learning to assess and analyze the educational needs of professional nurses related to breastfeeding and the use of breast milk, identifies educational objectives, plans and implements educational activities, and conducts a comprehensive evaluation of educational activities. Provides consultation and resources to Nurse Managers, Nurse Leaders and staff of the patient care areas and collaborates to identify learning needs. Documents aspects of learning activities in compliance with departmental, organizational and external agency requirements. Collaboration: Focuses on the multidisciplinary team when providing breastfeeding resources and support. Promotes community outreach activities to share the benefits of breastfeeding. Designs interdisciplinary educational curriculum and programs based on a variety of sources such as quality outcomes, trends and changes in breastfeeding and breast milk usage. Utilizes skilled oral, written and electronic communication to communicate effectively and to foster collegial relationships both intra- and interdepartmentally. Response to Diversity: Reviews alternative approaches and implements planned change to meet the breastfeeding mother’s needs considering cultural, social, family and system diversity. Clinical Inquiry: Individualizes standards and guidelines for particular patient situations. Questions and/or evaluates current practice based on patients’ responses, review of literature, research and education/learning. Maintains the knowledge and skills needed to address questions arising in practice and improve patient care.
