Evaluates the patient's and family's response to care, and modifies care plan accordingly. Assists patient and family in discharge planning so that problems and care needs are anticipated and met. A registered nurse responsible and accountable for assessing, planning and providing care to assigned patients. The Forensic Nurse Examiner is responsible for providing comprehensive care for patients who may have experienced sexual or physical assault or who have some other forensic need. This includes assessment, diagnosis, outcomes identification, planning, implementation and evaluation. The FNE functions independently and in collaboration with members of a multidisciplinary team to provide exceptional care to their patients. Essential Functions Clinical Judgment Meticulously document patient history utilizing patient quotations as much as possible. Perform photo documentation as needed. Demonstrate sound clinical judgment in the care of FNE patients as evidenced by peer review and case review of patient charts. Clinical Inquiry Obtain a medical and social history. Perform a complete head-to-toe assessment of the patient. Perform a detailed genital exam. Caring Practices Obtain a history of presenting concern utilizing non-leading feedback and open-ended questions. Acknowledge and address psycho-social needs of the patient and family pertinent to the patient’s plan of care. Response to Diversity Serve as a resource for pediatric forensic care throughout the Delaware Valley. Advocacy/Moral Agency Evaluate the need for collection of forensic evidence, and perform a meticulous collection of such evidence as needed. Maintain chain of custody for forensic evidence. Provide courtroom testimony. Collaboration Collaborate with the multidisciplinary team in the investigation of and response to sexual, emotional and physical trauma. Collaborate with attending physician, social worker and other members of the multidisciplinary team to develop a plan of care for the patient that meets their medico-legal, medical, emotional and social needs. Systems Thinking Refer the patient and family for short- and long-term follow-up of emotional, physical, and forensic or legal needs. Facilitation of Learning  Educate members of the hospital and medical teams about the role of the FNE in response to trauma. Provide education about child abuse within the hospital and community. Requirements Graduation from an approved school of nursing as a Registered Nurse. Registered Nurse in the state of Delaware. Minimum of 2 years experiencing working as an RN. Successful completion of an approved SANE A, SANE P training course and completion of clinical skills training requirements outlined by the International Association of Forensic Nurses (IAFN). Meets the requirements to sit for the SANE-Pediatric certification test offered by the IAFN Forensic Nursing Certification Board within 2 years of appointment. Preferred Experienced SANE nurse preferred. Pediatric experience preferred. Emergency department experience preferred. Summary The Case Management Nurse is a registered nurse who coordinates all aspects of the care of individual patients. Through the use of the nursing process, the patient care will be assessed, planned, implemented and evaluated with consideration to the appropriate use of resources. The Case Manager will manage care with a focus on designated clinical, operational and financial outcomes for aggregate patient populations. In collaboration with the interdisciplinary team and a family-centered process, the Case Management nurse will work to improve outcomes as measured by coordinating care to the correctly timed discharge, reduced readmission rates and improved patient/family satisfaction. Using the Standards for Case Management as a framework, will incorporate ethical and legal tenets into the essential job functions. Primary Responsibilities Conduct a comprehensive assessment of the patient’s health and psychosocial needs, including health literacy status and deficits, and develop a case management plan in collaboration with the family. Facilitate communication and coordination between members of the health care team, involving the family in the decision-making process in order to minimize fragmentation. Ensure correct level of utilization of services and resources for a select group of patients. Chart reviews are completed daily. Based on nationally recognized criteria, medical necessity for admission and continued stay is evaluated and presented to the team. Communicate with the payor liaison to ensure proper utilization of patient care resources in the hospital and in the community at the time of discharge. Additional Requirements The Case Manager oversees the delivery of care from admission through discharge and transition to the home environment. Family-centered assessment and discharge planning is started within 24 hours of admission. Documentation is done clearly and concisely in the patient chart. A plan of care with a path to home is developed with the interdisciplinary care team. Through a family-centered, interdisciplinary approach, the team can anticipate potential barriers and work together to ensure they are managed before delaying care, transition or discharge. Family education for care in the home is initiated by the direct care nurses and is kept on track. The Case Manager collaborates with the direct care nurses to evaluate progression of education and to surface any barriers to learning care. Anticipates discharge needs in terms of equipment, resources and follow-up care so that arrangements can be initiated as early in the course as possible. Case Managers work closely with Social Workers to ensure a safe and successful discharge. The Case Manager is able to reprioritize based on changing needs of the patient aggregate and plan accordingly. Education and Training Pediatric experience required, minimum of 5 years. BSN required. Delaware Nursing licensure required. Certification in Case Management preferred; attainment within 2 years of employment. This position is responsible for organizing, compiling, documenting and analyzing data to produce reports from which opportunities for quality/safety improvement, intervention and educational activities can be identified. Educates staff in continuous quality improvement and patient safety concepts, processes and tools as needed within the scope of these responsibilities. Utilizes the quality methodology in training departments and multidisciplinary teams. Works with hospital and medical staff at all levels of the organization to design and implement data collection methodologies, databases, action plans and reporting strategies. Facilitates teams as necessary to achieve performance improvement within the organization. Participates in continuous survey-readiness activities. Job Description: Registered Dietitian: Medical Nutrition Therapist (Part Time) Minimum Requirements: • Previous work experience with diet modifications in an acute care of long term care setting preferred. • Graduate of an accredited Bachelor's Program in Nutrition or related field. • Must have Excellent Customer Service Skills. • Successful passage of American Dietetic Association Registration Exam. • Minimum Bachelor’s degree from an accredited university or college and course work accredited or approved by the Academy of Nutrition and Dietetics (AND), passage of a national examination administered by the Commission on Dietetic Registration (CDR) and Licensed by the State of Delaware (LDN). • Previous work experience with medical nutrition therapy in an acute care or long term care setting preferred. Responsibilities include: collaboration with providers to identify and connect with patients in need of nutrition counseling, administers medical nutrition therapy as part of the health care team, establishes treatment plans and communicates with patients regarding program compliance, makes recommendations to primary care providers and assures compliance with standard practices and regulatory requirements. Applicants must have excellent customer service skills. Other duties may be assigned at the discretion of the supervisor. Position may include both outpatient and acute care inpatient assignments.
