Primary Function The Nemours Center for Cancer and Blood Disorders’ Cancer Care Coordinator (CCC) is responsible for developing, implementing and overseeing infrastructure and resources to enable comprehensive care for children with cancer through all phases and locations of care. The CCC will coordinate a multidisciplinary, patient/family-centered medical team ensuring the highest quality of care throughout the community and across the health care continuum. Essential Functions Completes a comprehensive assessment of patient at time of diagnosis, including, but not limited to, interview and screening of patient and family related to health status, patient/family educational needs, psychosocial status and anticipated home care needs. Refers patient and family to appropriate supportive resources as needed related to initial needs assessment. Continued facilitation of supportive services as needed throughout treatment course. Coordinates the development of clinical care plan for each patient in collaboration with multidisciplinary team. Communicates patient plan of care proactively with all involved members of the health care team, including the patient and family, on a daily basis. Coordinates initiation of patient/family education related to diagnosis, clinical care, ongoing treatment plan, anticipated long-term patient needs and clinical research studies. Establishes realistic short- and long-term goals activities to enhance patient self-management and participation of patient and family in plan of care. Monitors patient progress throughout course of treatment. Consults with primary oncologist and other members of the health care team to achieve appropriate patient care goals. Coordinates, in collaboration with the multidisciplinary team, second opinions as requested by patients/families and/or physicians. Promotes and provides patient, family, staff and community education through the use of clinical expertise and knowledge base. Develops innovative and state-of-the art education and assessment tools. Tracks patient outcomes and relevant data using appropriate information systems and pathways. Implements performance improvement programs for the care management of oncology patients. Monitors progress in achieving defined care management, clinical and administrative outcomes. Participates in hospital-wide quality and patient safety initiatives. Assumes responsibility for ongoing professional growth. Acts as a role model for professional nursing behaviors and clinical practice. Maintains confidentiality of patients’ health care information as per hospital policy. Attends family meetings,  Hematology/Oncology Inpatient Rounds, Outpatient Rounds, Tumor Board and other educational forums as needed. Organizes, coordinates and facilitates Oncology Family Advisory Council meetings; disseminates lessons learned to the appropriate parties for action. Responsible for participation in departmental and hospital programs for continuous improvement, identifying opportunities for practice improvement and increased patient/family satisfaction, making recommendations and implementing actions as appropriate and consistent with the goals of the Nemours Foundation. Primary Function This position is part of a newly formed Patient Navigation Team responsible for the delivery of coordinated care and scheduling for patients and families requiring outpatient subspecialty services from multiple departments. The primary functions include: Triaging all inquiries/requests from patients/parents and referring physicians (and their office staff) for complex appointments (via phone, email, in-basket orders or messages). Coordinating care and providing guidance and education to the family to ensure the patient receives the necessary care. Registering and scheduling or facilitating scheduling of these patients with the appropriate providers. Addressing clinical and non-clinical inquiries according to established divisional protocols and guidelines. Essential Functions Receive intake from patients/parents, families, physician office representative and schools for triage; assess urgency for evaluation and management of care for outpatient appointments. Provide clinical assessment expertise to scheduling parameters – timing of visit, appropriate provider, etc. Collaborate with clinical provider as necessary. Serve as an educator/resource to the Patient Navigation Team on divisional protocols for outpatient appointment scheduling, patient triage, provider expertise and other details that are pertinent to accurate appointment scheduling. Participate and represent Patient Navigation Team in department programs and meetings. Communicate (by preferred method) all procedure-related preparation information to families directly during the scheduling process. Track patients and facilitate coordination and continuity of care. Work collaboratively with division members relative to maintenance and upkeep of current divisional scheduling protocols. When necessary, create correspondence in response to a patient request for clinical provider review. Serve as a liaison for communicating patient care plans between all stakeholders (i.e., PCP, specialists, families, etc.). Proactively develop and enhance processes with the goal of improving the clinical experience for referred patients and the referring physician. Maintain all necessary licenses, education, etc. Participates in program, hospital-wide CQI projects. Complete mandatory in-service training yearly that includes, but is not limited to, safety, infection control, HIPPA, corporate compliance and bioterrorism. Maintain data and related outcomes to complex scheduling. Collaborate with marketing and outreach departments to educate referring physicians and facilities regarding complex scheduling services. Licensing/Educational Requirements Current, unrestricted DE and PA Registered Nurse licenses required; ability to obtain additional state licensing, if necessary. Bachelor of Science in Nursing required; active matriculation toward BSN if not already achieved. Care management experience preferred. Experienced pediatric nurse; NICU experience a plus. Functions in the role of the Incident Commander during a Code Delta until relieved by the Administrator-on-Call. Manages and resolves all patient care issues in the absence of the Chief Nurse Executive/designee. Identifies problems, gathers sufficient data, analyzes data and determines strategies to utilize for problem resolution. Contributes appropriately to quality improvement activities. Evaluates outcome of problem resolution to determine effectiveness. Supports development of staff by encouraging participation in educational programs. Identifies individual and unit educational needs for a specific shift on weekends and communicates to Nurse Managers/designee on Nursing Staff Development. Communicates staff deficiencies to the unit Nurse Manager. Provides periodic evaluation of staff to the unit Nurse Manager. Provides constructive guidance to staff for development of clinical expertise. Counsels employees in the absence of the Nurse Manager. Demonstrates ability to work collaboratively with ancillary departmental staff. Establishes a positive work environment to achieve optimal patient care. Demonstrates effective and professional written and oral communication skills. Conveys unit information regarding patients, unit activity, staffing and personnel problems to the Nurse Manager/designee daily. Provides recommendation to improve nursing workflows and efficiency. Requirements BSN required. Graduate of accredited school of nursing. Active Delaware (or compact state) Registered Nurse licensure required. 3-5 years experience as a staff nurse. Facilitates workflow, including staff assignments congruent with their abilities and the patient care requirements. Rapidly prioritizes demands with resources to meet individualized patient needs. Anticipates patient's needs as they relate to special equipment, support services, vendors and scheduling of procedures. Competency Demonstrates knowledge in the specialty area. Collaborates with anesthesia care providers and surgeons to determine intra-op needs of the patient to include equipment, instrumentation, positioning and scheduling. Works with the Assistant Nurse Manager (ANM) to enhance daily workflow, including staff assignment congruent with staff's abilities and patient care requirements. Partners with the Clinical Nurse Specialist (CNS) to assess the educational needs of the OR, and presents service-specific and unit-specific in-services to provide continued learning for staff members. Collaborates with the entire health care team (ANM, CNS and Nurse Manager) to orient new staff members. Communicates with other units to provide continuity of patient care when transferring to and from the operating room. Engages ancillary departments to assist in the provision of appropriate supplies, instruments and equipment for safe patient care. Role Model/Leadership Acts as a coordinator motivating staff with the goal of empowering all staff members to function independently, aspiring to excellence. Prepares agenda/goals, provides appropriate material, and keeps minutes when conducting meetings. Patient and family care needs are assessed, interventions provided and outcomes evaluated based upon written departmental policies and procedures, as well as multidisciplinary standards of care and practice. Personnel providing care are: The Division of Pediatrics with board-certified pediatricians and pediatric subspecialists and residents The Division of Gastroenterology The Division of Endocrinology The Division of Pulmonology Advanced Practice Nurses are available and work collaboratively with service physicians. The Nurse Manager has 24-hour accountability for the management and leadership of the staff. The Assistant Nurse Manager reports directly to the Nurse Manager, provides both direct and indirect patient care, and is responsible for the daily coordination of unit activities. The Medical Nurse Clinicians assist the Nursing leadership team with planning for educational opportunities, including orientations and skills reviews, as well as promoting professional development of all nursing staff. Registered Nurses provide direct patient care in a variety of scheduling patterns to meet the needs of the unit. They perform assessment, planning, implementing and evaluating care of the patient population of the unit; collaborate with other departments to facilitate care; and are actively involved in daily rounds, patient/family education and discharge planning. Staff participates in unit and hospital-based Shared Governance committees supporting the Drive to Excellence. Nursing Assistants perform established aspects of patient care under the direct supervision of a Registered Nurse. They also assist staff and families in facilitating communications with other units and departments of the hospital. A complete complement of ancillary services including, but not limited to, Laboratory, Radiology, Respiratory, Therapeutic Services, Nutrition Services, Child Life and Social Work. The patient care complexity directly relates to the wide range of patient care skills required for this diverse patient population. The goal of this unit is to provide high-quality, cost-effective patient care. The Synergy model is used in making appropriate patient assignments based on patient and family needs and nursing skill set. Special equipment used on this unit includes infusion pumps, syringe pumps, feeding pumps, BiPAP respirators, oxygen delivery methods, PCA and epidural pumps, wound vacutainers, ventilators and specialty beds. Nursing competencies include chest tube management, pain management, blood product administration, urinary catheterization, tracheostomy care, ventilator management and central line care and management. Requirements We are looking for professional and dedicated nurses. BSN Nurses preferred. Pediatric experience. At least 6 months of nursing experience preferred. Bilingual preferred. Monitor chronic/preventive patient registries/lists. Initiate family contacts; create ongoing processes for families to determine and request the level of care-coordination support they desire for their child/youth or family member at any given point in time. May facilitate or assist with Family Advisory Committee. Identify patient and family needs and unmet needs, strengths and assets. Assess biopsychosocial needs of at-risk patients, i.e., single parents, substance abuse, complex medical patients, etc. Build care relationships among family and team; support the primary caregiving role of the family. Promote teams and actively participate in daily huddles. As a member of the care team, develop and implement patient care plans with family/youth/team (emergency plan, medical summary and action plan as appropriate). Carry out care plans, evaluate effectiveness, monitor in a timely way, and effect changes as needed; use age-appropriate transition timetables for interventions within care plans. Contact identified patients for preventative services and/or pre-visit forms. Case management coordination of services such as transportation, referrals, post-hospitalization discharge and compliance – especially chronic medications. Arrange for supplies and equipment. Assist with getting insurance coverage for patients without insurance. Coordinate inter-organizationally among family, the medical home and involved agencies. Identify community resources and track select community and specialty referrals. Connect to and understand community resources, i.e., WIC, food stamps, DME providers, advocacy groups, schools, financial assistance, counseling, anger management classes, special needs camps or inner-city camps. Refer patients to early intervention and public health nurses, help office staff and parents navigate through the school system, and help with IEPs. Promote/document Quality Improvement Cycles. Responsible for generating required data. Facilitate the NCQA process at the office working in close collaboration with the Medical Home liaison. Non-Essential Functions Facilitate family access to medical home providers, staff and resources. Actively participate on office teams. Help educate patients on using medical services appropriately; improve patient/family health care literacy, especially for at-risk patients. Work with chronic no-show patients to identify issues related to not keeping appointments. Educate, counsel and support. Provide developmentally appropriate anticipatory guidance; in a crisis, intervene or facilitate referrals appropriately. Counsel support with brief interventions/help run support groups. Serve as a medical home quality improvement team member; help to measure quality and to identify, test, refine and suggest practice improvements; download report, build spreadsheets and analyze data. Cultivate and support primary care and subspecialty co-management with timely communication, inquiry, follow-up and integration of information into the care plan. Coordinate efforts to gain family/youth feedback regarding their experiences with health care (focus groups, surveys, other means); participate in interventions that address family/youth articulated needs. We recently completed a multi-phase hospital expansion that includes new inpatient rooms, Pediatric Intensive Care Unit and Emergency Department. Additionally, Nemours duPont Pediatrics allows us to reach more children across the region through community-based physician services and collaborative partnerships with health and hospital systems. 2 West is a surgical intermediate unit with a high population of patients requiring post-operative care for urological issues, traumatic injuries and general surgery. 2 West also cares for patients with acute and end stage renal disease and patients requiring an intermediate level of nursing care due to their tracheostomy and/or ventilator needs. Each patient room is equipped with oxygen, compressed air and suction, along with state-of-the-art cardiorespiratory bedside monitors and computers that enable the health care team to access a patient’s electronic medical record. The patient care complexity directly relates to the wide range of patient care skills required for this diverse patient population. The goal of this unit is to provide high-quality, cost-effective patient care. The Synergy model is used in making appropriate patient assignments based on patient and family needs and nursing skill set. Special equipment used on this unit includes infusion pumps, syringe pumps, feeding pumps, BiPAP respirators, oxygen delivery methods, PCA and epidural pumps, wound vacutainers, ventilators and specialty beds. Nursing competencies include chest tube management, pain management, blood product administration, urinary catheterization, tracheostomy care, ventilator managemen and central line care and management. Requirements BSN nurses required. 1 year acute care experience required. Trach/vent experience preferred. Pediatric experience preferred. Bilingual preferred. Must be eligible for or have current DE nursing license. Must have CPR certification through the American Heart Association. The Neonatal Intensive Care Unit (NICU) of the Alfred I. duPont Hospital for Children is a referral center primarily for infants less than 30 days of age requiring surgery. The NICU is capable, however, of caring for all infants needing intensive care, except for those requiring extracorporeal membrane oxygenation (ECMO). Principle diagnoses include abdominal wall defects, necrotizing entercolitis, neural tube defects, hydrocephalus, urologic defects and infants with congenital anomalies. The NICU functions 24 hours a day, seven days a week. It is supported by Thomas Jefferson University. The NICU is a state-of-the-art, fourteen-single room unit. The Neonatal Intensive Care Unit (NICU) of the Alfred I. duPont Hospital for Children is a referral center primarily for infants less than 30 days of age requiring surgery. The NICU is capable, however, of caring for all infants needing intensive care, except for those requiring extracorporeal membrane oxygenation (ECMO). Principle diagnoses include abdominal wall defects, necrotizing entercolitis, neural tube defects, hydrocephalus, urologic defects and infants with congenital anomalies. The NICU functions 24 hours a day, seven days a week. It is supported by Thomas Jefferson University. The NICU is a state-of-the-art, fourteen-single-room unit. The Pediatric Intensive Care Unit of the Alfred I. duPont Hospital for Children is a 22-bed unit in a Level 1 Trauma Center. Patients requiring care and surveillance in the PICU will have conditions and diagnoses that may include, but are not limited to, those producing: acute and reversible loss of consciousness, respiratory failure, compromised air exchange, cardiovascular dysfunction, hepatic failure, hemodynamic instability, renal dysfunction, central nervous system injury or dysfunction, acute trauma and organ failure requiring transplantation. Treatments or interventions performed in the PICU include measures utilizing advanced life support technology. These include, but are not limited to, cardiopulmonary resuscitation, tracheal intubation, ECMO, pharmacologic support of circulation, mechanical ventilation, point-of-care monitoring, CRRT, thoracentesis and hemodynamic, intracranial pressure and intravascular monitoring. Each patient room has electrocardiographic, pulse oximetry, respiratory, hemodynamic and intracranial pressure monitoring capability, as well as central EKG and respiratory monitoring. Portable monitoring during patient transport is provided. Special equipment includes radiant warmer beds, heating/cooling blanket systems, Mapleson manual ventilation system, end tidal CO2 monitors, ventilators, hemodynamic monitoring systems, intracranial pressure monitoring system, ventriculostomy systems, infusion pumps, blood warmer, continuous renal replacement systems and peripheral nerve stimulators. Requirements Be a graduate of an accredited school of nursing; BSN preferred. Must be an experienced pediatric intensive care nurse of at least 2 years. Have a current registration or be for eligible Delaware State Board of Nursing Licensure. Must be CPR-certified by the American Heart Association; PALS preferred. Certifications: CCRN preferred. The department is composed of 17 monitored emergency treatment rooms, a 6-bed Fast Track area handling non-urgent patients during defined hours, and a 6-bed monitored observation area. All required advanced and basic emergency life support equipment and supplies are maintained within specific rooms and within the department. We recently completed a multi-phase hospital expansion that includes new inpatient rooms, Pediatric Intensive Care Unit and Emergency Department. Additionally, Nemours duPont Pediatrics allows us to reach more children across the region through community-based physician services and collaborative partnerships with health and hospital systems. The Advanced Practice Nurse is responsible for an area of specialized nursing practice and provides direct and indirect care to patients and their families in an outpatient primary care setting. Essential Responsibilities Practices collaboratively with physicians and other health care providers to provide high-quality health care to children. Performs appropriate assessment of patient/family. Performs ongoing assessment and evaluation of patient progress and revision of interdisciplinary plan of care. Identifies patient/family need for further resources, and initiates appropriate referrals. Documents patient care and progress on patient’s chart in electronic medical record. Provides patient/family teaching based on identification of learning needs. Evaluates compliance with the prescribed treatment regimen through telephone calls, family/patient assessments and outpatient visits as needed. Utilizes advanced knowledge and specialized skills in nursing practice in accordance with the Nurse Practice Act, hospital policy and protocol. Position Qualifications Education, Training and Experience Master of Science in Nursing. Five years of related clinical experience. Pediatric primary care experience preferred. Bilingual preferred. Licenses, Registrations or Certifications Current Registered Nurse license in the state of Delaware. Current Advanced Practice Nurse license in the state of Delaware with prescriptive authority.
