Christiana Care's Wound Care Center is seeking a Clinical Coordinator who will be responsible for coordination of clinical activites of the Wound Care Center including direct supervision to clinical staff.  Collaborates with physicians and other members of the healthcare team to provide consistent quality care for patients with wounds.  Case manages a group of patients by planning, implementing and evaluating plan(s) of care. Will provide direct care for patients in an ambulatory setting through comprehenisve assessments according to protocols and procedures, assessment of diagnostic data and wound evlauations, clinical competence in procedureal skils, and patient/caregiver education related to wound care management. Provides Hyperbaric Oxygen Therapy patient assessment and chamber operation as required. Requires: RN with current state license in good standing, BSN preferred 2 years' previous supervisory experience required 2-5 years wound care experience in an acute care, home care, ambulatory care and/or long term care setting. Certification in wound care desirable and/or equivalent years of wound care experience and training. Our patient population ranges from newborns to young adults with congenital or acquired heart disease. We perform all of the complex cardiac repairs, including heart transplants, utilizing the full range of technical modalities such as ECMO/ECLS and ventricular assist devices, including the Berlin Heart, Thoratec and implantable Heart ware models. We also provide nitric oxide and continuous renal replacement when indicated. Essential Responsiblities Obtain medical history, perform physical exam, and order and interpret laboratory data on all patients being admitted to the Nemours Cardiac Center. Assist the physician staff of the Nemours Cardiac Center to provide inpatient and outpatient care services. Assist the Cardiac Staff in the pre-operative preparation and post-operative care of surgical patients. Coordinate ongoing overall patient care with physician staff, other nurse practitioners and physician assistants, nursing staff and other ancillary care providers to ensure treatment plans are implemented. Interact with families of the patients on the cardiac service, embracing our family-centered focus. Write a daily progress note on inpatients, collate inpatient records and perform discharge summaries, utilizing the electronic medical record. Participate in departmental quality improvement initiatives. Attend departmental patient care conferences. Position Qualifications Education, Training and Experience Master of Science in Nursing. 5 years related clinical experience. Licensure, Registrations or Certificates Must have or be eligible for registered nurse in the state of Delaware. Must have or be eligible for Advanced Practice Nurse License in the state of Delaware with prescriptive authority. Develops, supports and facilitates hospital direction and goals. Contributes to professional growth and development of patient care team by identifying educational resources and opportunities with regard to clinic services to patients. Applies supervisory skills in a fair, consistent and objective manner. Collaborates with patient care team to monitor, evaluate and improve the patient care delivery system to increase customer satisfaction and clinic productivity. Assists in the planning, development, implementation and evaluation of policies, procedures and new services. Some travel is required to various Cardiac Center satellite offices in PA, NJ, DE and MD. Requirements BSN required. 3-5 years of nursing leadership experience preferred. 3-5 years pediatric cardiac experience required; outpatient experience preferred. The team operates 24 hours a day, seven days a week. Team consists of two team members, one of which is an RN. Direct medical control is supplied by board-certified pediatric intensivists. The majority of patients transported are under two years of age with principal admitting diagnoses of respiratory illness and neurological disorders. Trauma patients with both single-extremity and multi-system injuries are also served. The team uses specially-designed equipment to maintain or exceed the level of care provided by referring facilities, including multi-parameter vital-sign monitors, compact infusion pumps and defibrillator/external pacemakers. Advanced procedures performed by PTT staff include vascular access (intravenous and intraosseous), endotracheal intubation, chest decompression and advanced airway management. The PTT transports on average four patients daily with little seasonal variation in census. Ninety percent of patients are transported by ambulance; the remaining patients are flown by helicopter. Patient placement includes specialty units such as pediatric intensive care, cardiac intensive care, burn, emergency and surgical units, as well as general pediatric floors. In addition to performing inter-hospital transports, PTT staff assist with patient care in the PICU, NICU and ER, and serve as resources to other nursing units within the hospital. Looking for experienced nurses.   Must have critical care experience. Transport and pediatric critical care experience preferred. Bachelor's in Nursing preferred. Bilingual preferred. Must maintain or be eligible for licensure in the state of DE, PA and NJ. Additionally, Nemours duPont Pediatrics allows us to reach more children across the region through community-based physician services and collaborative partnerships with health and hospital systems. 2 West is a surgical intermediate unit with a high population of patients requiring post-operative care for urological issues, traumatic injuries and general surgery. 2 West also cares for patients with acute and end stage renal disease and patients requiring an intermediate level of nursing care due to their tracheostomy and/or ventilator needs. Each patient room is equipped with oxygen, compressed air and suction, along with state-of-the-art cardiorespiratory bedside monitors and computers that enable the health care team to access a patient’s electronic medical record.  The patient care complexity directly relates to the wide range of patient care skills required for this diverse patient population. The goal of this unit is to provide high-quality, cost-effective patient care. The Synergy model is used in making appropriate patient assignments based on patient and family needs and nursing skill set. Special equipment used on this unit includes infusion pumps, syringe pumps, feeding pumps, BiPap respirators, oxygen delivery methods, PCA and epidural pumps, wound vacutainers, ventilators and specialty beds. Nursing competencies include chest tube management, pain management, blood product administration, urinary catheterization, tracheostomy care, ventilator management and central line care and management. We are looking for professional and dedicated nurses. BSN nurses preferred. Pediatric experience. At least 6 months of nursing experience preferred. Bilingual preferred. Must be eligible for or have current DE nursing license. Must have CPR certification through the American Heart Association. The nursing staff continuously monitors and assesses patients during their stay. They also assess the learning needs of the parent/caregiver. Parents/caregivers are united with their child as soon as possible after surgery, giving the nurse the opportunity to provide education on the care of the child going home following recovery or on what to expect for the child being admitted or returned to a nursing unit. The nursing staff works closely with the surgeons and the anesthesia care providers to guide the child safely through the postoperative period. A. Education, Training and Experience Graduate of accredited school of nursing, BSN preferred. Must be an experienced Nurse. Previous experience in PACU, pediatric ICU nursing preferred. Job Description: Nurse, RN, Neonatal Intensive Care, 7pm - 7am (NICU, Nursing, Nights) Nemours is seeking a Registered Nurse in our NICU to join our team in Delaware.  This opening is 7pm-7am, 72 hours every two weeks with weekend and holiday rotation.   As one of the nation's premier pediatric health care systems, Nemours provides world-class clinical care in four states: Delaware, Florida, New Jersey and Pennsylvania. Upon completion in June 2014, the hospital will include private patient rooms, a new Pediatric Intensive Care Unit, and a new expanded Emergency Department. The Neonatal Intensive Care Unit (NICU) of the Alfred I. duPont Hospital for Children is a referral center primarily for infants less than 30 days of age requiring surgery. The NICU is capable, however, of caring for all infants needing intensive care, except for those requiring extracorporeal membrane oxygenation (ECMO). Principle diagnoses include: abdominal wall defects, necrotizing entercolitis, neural tube defects, hydrocephalus, urologic defects, and infants with congenital anomalies. The NICU functions 24 hours a day, seven days a week. The NICU is a collaborative effort of the Alfred I. duPont Hospital for Children and the Christiana Care Health System (CCHS). It is supported by Thomas Jefferson University. The NICU is a state of the art fourteen single room unit. Four rooms can flex to accommodate two infants, for a total capacity of 18 infants.  Each bed is monitored and supported by neonatal nurses, neonatal nurse practitioners, neonatologists, pediatric surgeons and pediatric sub-specialists. Staffing is well supported with ratios of 1:2-3. Requirements You must be a graduate of an accredited school of nursing. Must be an experienced nurse, NICU expereince preferred. Bilingual Preferred Previous experience in pediatric critical care experience nursing preferred. Have a current registration or be eligible for Delaware State Board of Nursing Licensure. Job Description: Telephone Triage Nurse, RN, Casual, Wknds, Eves & Nights (BSN, Pediatrics) Nemours is seeking a Telephone Triage Nurse to join our team in Wilmington, DE. This position is variable shifts, weekday evenings, weekends day shift, evenings and overnights. While on orientation will be required to work weekdays 4pm-12am and weekends for the first 6-12 weeks. Applicant should be flexible to pick up extra shifts when needed and be available to work evenings, weekends and holidays as this unit serves our patients after hours. The KidsHealth On-Call telephone nurse triage unit provides patients with telephone access to a highly skilled and trained registered nurse during hours when the Nemours primary care offices are closed. Job functions include: Accurately assessing each patient's condition by actively listening to callers and asking probative questions. Identifying emergent situations and effectively transferring life-threatening calls to appropriate agencies per protocol. Providing appropriate and courteous telephone advice, including plan of care and patient education, to callers according to approved protocols. Appropriately and accurately documenting initial and ongoing telephone conversations with documentation being completed immediately after the call ends. Placing outbound follow-up calls to caregivers as needed. Accurately communicating information to physician on call as needed either over the telephone or by routing the necessary information to the physician through Epic. Meeting unit expectation for performance based on unit competencies and staff meeting attendance. Must be able to work independently and to make critical decisions autonomously, as this is a complete work-at-home position after completion of orientation. Requirements Candidate should possess excellent communication and listening skills. Ability to multitask, make time-sensitive decisions and function well independently. Must be able to work independently from home in a private workspace. Current PA, DE and NJ nursing licenses or ability for license reciprocity in these states. RN required; BSN preferred. 3-5 years pediatric experience; nurse triage or primary care nursing experience preferred. Bilingual, Spanish-speaking skills are a plus. Experience with electronic medical records (EMR) is a plus. Orientation will be weekdays 4pm-12am and weekends. Applicant should be flexible to pick up extra shifts when needed and be available to work evenings, weekends and holidays as this unit serves our patients after hours. Work hours: Flexibility to work nights, weekends, holidays and evenings. Some overnights are required. Shifts may be adjusted due to department needs. Casuals must work a minimum of 32 hours in a 6-week period and a minimum of 5 midnight shifts per year. Must live within a 60-minute commute of the Nemours/Alfred I. duPont Hospital. Although this is a work-from-home position once orientation is completed, there may be times you will be required to work from the hospital or attend mandatory hospital-based meetings.
