Primary Function The Diabetes/Endocrine Nurse Educator is responsible for an area of specialized nursing practice and provides direct and indirect care to patients and their families in the outpatient setting. Essential Responsibilities Performs appropriate assessment of patient/family. Performs ongoing assessment and evaluation of patient progress and revision of interdisciplinary plan of care related to diabetic/endocrine patient needs. Identifies patient/family need for further resources, and initiates appropriate teaching and/or referrals for diabetic/endocrine patients. Assists direct care nurse in formulating the patient care plan and setting priorities based on the needs of patient and family. Supports the nurse in teaching performed. Responds to patient phone calls and participates in phone triage. Education, Training and Experience Bachelor of Science in Nursing. Three years of pediatric and related clinical experience. Licenses, Registrations or Certifications Current license in the State of Delaware. Primary Function The Nemours Center for Cancer and Blood Disorders’ Cancer Care Coordinator (CCC) is responsible for developing, implementing and overseeing infrastructure and resources to enable comprehensive care for children with cancer through all phases and locations of care. The CCC will coordinate a multidisciplinary, patient/family-centered medical team ensuring the highest quality of care throughout the community and across the health care continuum. Essential Functions Completes a comprehensive assessment of patient at time of diagnosis, including, but not limited to, interview and screening of patient and family related to health status, patient/family educational needs, psychosocial status and anticipated home care needs. Refers patient and family to appropriate supportive resources as needed related to initial needs assessment. Continued facilitation of supportive services as needed throughout treatment course. Coordinates the development of clinical care plan for each patient in collaboration with multidisciplinary team. Communicates patient plan of care proactively with all involved members of the health care team, including the patient and family, on a daily basis. Coordinates initiation of patient/family education related to diagnosis, clinical care, ongoing treatment plan, anticipated long-term patient needs and clinical research studies. Establishes realistic short- and long-term goals activities to enhance patient self-management and participation of patient and family in plan of care. Monitors patient progress throughout course of treatment. Consults with primary oncologist and other members of the health care team to achieve appropriate patient care goals. Coordinates, in collaboration with the multidisciplinary team, second opinions as requested by patients/families and/or physicians. Promotes and provides patient, family, staff and community education through the use of clinical expertise and knowledge base. Develops innovative and state-of-the art education and assessment tools. Tracks patient outcomes and relevant data using appropriate information systems and pathways. Implements performance improvement programs for the care management of oncology patients. Monitors progress in achieving defined care management, clinical and administrative outcomes. Participates in hospital-wide quality and patient safety initiatives. Assumes responsibility for ongoing professional growth. Acts as a role model for professional nursing behaviors and clinical practice. Maintains confidentiality of patients’ health care information as per hospital policy. Attends family meetings,  Hematology/Oncology Inpatient Rounds, Outpatient Rounds, Tumor Board and other educational forums as needed. Organizes, coordinates and facilitates Oncology Family Advisory Council meetings; disseminates lessons learned to the appropriate parties for action. Responsible for participation in departmental and hospital programs for continuous improvement, identifying opportunities for practice improvement and increased patient/family satisfaction, making recommendations and implementing actions as appropriate and consistent with the goals of the Nemours Foundation. Communicates with other units to provide continuity of patient care when transferring to and from the operating room. Engages ancillary departments to assist in the provision of appropriate supplies, instruments and equipment for safe patient care. Job Description: Nurse, RN, Ortho / Acute Rehabilitation, Rotating Shifts (Pediatrics, BSN) Nemours is seeking a Registered Nurse - Ortho/Acute Rehabilitation to join our team in Wilmington, DE. This position is 60 hours every two weeks, rotating shifts with every third weekend and holiday rotation. This unit is designated for acute rehabilitation of both traumatic brain injury and spinal cord injury, comprehensive rehabilitation and ventilator-dependent rehabilitation patients with tracheotomies. Additional services include orthopedic surgical care, medically complex pediatric patients, transitional neonatal care, neurosurgery and non-surgical neurologic care, as well as video EEG studies. Special equipment includes ventilators, portable suction, feeding pumps, BIPAP and CPAP machines, infusion pumps and wound vacutainers. Each bed is also equipped with bedside monitors and computers, compressed air, oxygen and vacuum hook-up for suction. Patient care unit goals are to provide high-quality, cost-effective care. Patient- and family-centered care is the nursing model used to provide care for all patients. Patients’ and families’ needs are assessed, interventions provided and outcomes evaluated based upon departmental policies and procedures, as well as multidisciplinary standards of care and practice. This unit also provides education to families and caregivers to ensure they are comfortable caring for their child. The unit participated in a CARF survey in 2011 in which the Rehabilitation Program received a full accreditation. Individuals and services providing care on 3 West: The Divisions of Orthopedic Surgery and Pediatrics both with board-certified physicians, fellows and residents. The Division of Rehabilitation, consisting of 3 board-certified physiatrists/pediatricians. This division also provides residents and program managers. The divisions of Neonatology, Neurology, Neurosurgery and Pulmonology. Advanced Practice Nurses are available for all patients and work collaboratively with the appropriate service physician. A nurse manager has 24-hour accountability for management and leadership of the staff. A registered nurse assumes a charge nurse role on all shifts and is responsible for the coordination of unit activities and the evaluation, anticipation and communication with the nursing supervisor. Unit clerks/nursing assistants assist the staff and families in facilitating communications with other units and departments of the hospital.  They may also perform limited aspects of patient care under the direction of a registered nurse. A complete complement of ancillary services including, but not limited to, laboratory, radiology, respiratory, therapeutic services, nutrition, social work and pharmacy. The Rehabilitation Team provides a multidisciplinary approach to Rehabilitation patients. A board-certified physician directs the team. Registered nurses providing care are BLS, PALS and ENPC-certified. Other certifications include CPN and CRRN. Performs back office laboratory tests. Administers immunizations, oral and/or injectable medications. Administers PPD and interprets results. Orders and stocks appropriate inventory of medical-surgical supplies for lab and exam rooms. Triages patient calls utilizing delineated protocols under the supervision of the provider. Provides age- and developmentally appropriate care to infants, children, adolescents and adults. Monitors patient flow, including preparing patients and rooms for physicals and sick visits. Administers a variety of allergy testing, including skin testing, intradermal, patch testing and immunotherapy. The goal of this unit is to provide high-quality, cost-effective patient care. The Synergy model is used in making appropriate patient assignments based on patient and family needs and nursing skill set. Special equipment used on this unit includes infusion pumps, syringe pumps, feeding pumps, BiPAP respirators, oxygen delivery methods, PCA and epidural pumps, wound vacutainers, ventilators and specialty beds. Nursing competencies include chest tube management, pain management, blood product administration, urinary catheterization, tracheostomy care, ventilator management and central line care and management. Delaware RN license or Compact License. 5 years of related experience and strong clinical skills required. Dysrhythmia certification required. CPR through the AHA. The Nemours Cardiac Center (NCC) provides comprehensive care for infants, children and adolescents with cardiac, thoracic and vascular disease. The Cardiac Center provides patient care, performs research, and educates and trains health care professionals consistent with the Mission, Vision and Values of the Nemours Foundation. Essential Responsibilities: Assesses and documents initial and ongoing patient needs and problems. Formulates nursing plan based on patient needs and problems, and indicates plan on the patient care plan. Develops realistic and appropriate long- and short-term goals based on patient/family needs. Implements patient plan of care. Evaluates the patient's and family's response to care, and modifies care plan accordingly. Assists patient and family in discharge planning so that problems and care needs are anticipated and met. Serves as an advocate for patient and family in working with other members of the health care team. Provides safe and appropriate nursing care through adherence to Nursing Department standards. A registered nurse responsible and accountable for assessing, planning and providing care to assigned patients. Education, Training and Experience Graduate of accredited school of nursing. Must be an experienced nurse. Bilingual preferred. Experience in pediatric/cardiac and or critical care nursing preferred. The Neonatal Intensive Care Unit (NICU) of the Alfred I. duPont Hospital for Children is a referral center primarily for infants less than 30 days of age requiring surgery. The NICU is capable, however, of caring for all infants needing intensive care, except for those requiring extracorporeal membrane oxygenation (ECMO). Principle diagnoses include abdominal wall defects, necrotizing entercolitis, neural tube defects, hydrocephalus, urologic defects and infants with congenital anomalies. The NICU functions 24 hours a day, seven days a week. It is supported by Thomas Jefferson University. The NICU is a state-of-the-art, fourteen-single-room unit. Four rooms can flex to accommodate two infants for a total capacity of 18 infants. Each bed is monitored and supported by neonatal nurses, neonatal nurse practitioners, neonatologists, pediatric surgeons and pediatric sub-specialists. Staffing is well supported with ratios of 1:2-3. Requirements You must be a graduate of an accredited school of nursing. Must be an experienced nurse; NICU experience preferred. Bilingual preferred. Experience in pediatric critical care experience nursing preferred. Have a current registration or be eligible for Delaware State Board of Nursing Licensure.
