Job Description: Advanced Practice Nurse, MSN, Cardiac Center, Rotating (CRNP, NP, Cardiology) Nemours is seeking an Advanced Practice Nurse for the Cardiac Center to join our team in Wilmington, DE. Primary Function This individual will work collaboratively with physicians, consultants, nurses and other health care practitioners, dedicating his/her specialized practice to patients in the CICU, inpatient floor and occasionally in the cardiac outpatient clinic. Our patient population ranges from newborns to young adults with congenital or acquired heart disease. We perform all of the complex cardiac repairs, including heart transplants, utilizing the full range of technical modalities such as ECMO/ECLS and ventricular assist devices, including the Berlin Heart, Thoratec and implantable Heart ware models. We also provide nitric oxide and continuous renal replacement when indicated. Essential Responsiblities Obtain medical history, perform physical exam, and order and interpret laboratory data on all patients being admitted to the Nemours Cardiac Center. Assist the physician staff of the Nemours Cardiac Center to provide inpatient and outpatient care services. Assist the Cardiac Staff in the pre-operative preparation and post-operative care of surgical patients. Coordinate ongoing overall patient care with physician staff, other nurse practitioners and physician assistants, nursing staff and other ancillary care providers to ensure treatment plans are implemented. Interact with families of the patients on the cardiac service, embracing our family-centered focus. Write a daily progress note on inpatients, collate inpatient records and perform discharge summaries, utilizing the electronic medical record. Participate in departmental quality improvement initiatives. Attend departmental patient care conferences. Position Qualifications Education, Training and Experience Master of Science in Nursing. 5 years related clinical experience. Licensure, Registrations or Certificates Must have or be eligible for registered nurse in the state of Delaware. Must have or be eligible for Advanced Practice Nurse License in the state of Delaware with prescriptive authority. The team operates 24 hours a day, seven days a week. Team consists of two team members, one of which is an RN. Direct medical control is supplied by board-certified pediatric intensivists. The majority of patients transported are under two years of age with principal admitting diagnoses of respiratory illness and neurological disorders. Trauma patients with both single-extremity and multi-system injuries are also served. The team uses specially-designed equipment to maintain or exceed the level of care provided by referring facilities, including multi-parameter vital-sign monitors, compact infusion pumps and defibrillator/external pacemakers. Advanced procedures performed by PTT staff include vascular access (intravenous and intraosseous), endotracheal intubation, chest decompression and advanced airway management. The PTT transports on average four patients daily with little seasonal variation in census. Ninety percent of patients are transported by ambulance; the remaining patients are flown by helicopter. Patient placement includes specialty units such as pediatric intensive care, cardiac intensive care, burn, emergency and surgical units, as well as general pediatric floors. In addition to performing inter-hospital transports, PTT staff assist with patient care in the PICU, NICU and ER, and serve as resources to other nursing units within the hospital. Looking for experienced nurses.   Must have critical care experience. Transport and pediatric critical care experience preferred. Bachelor's in Nursing preferred. Bilingual preferred. Performs back office laboratory tests. Administers immunizations, oral and/or injectable medications. Administers PPD and interprets results. Orders and stocks appropriate inventory of medical-surgical supplies for lab and exam rooms. Triages patient calls utilizing delineated protocols under the supervision of the provider. Provides age- and developmentally appropriate care to infants, children, adolescents and adults. Monitors patient flow, including preparing patients and rooms for physicals and sick visits. Administers a variety of allergy testing, including skin testing, intradermal, patch testing and immunotherapy. Work hours: Monday-Friday, hours may vary from 8:00am to 8:00pm. Must be flexible with work hours.   Requirements Pediatric experience preferred. Bilingual candidates preferred. May facilitate or assist with Family Advisory Committee. Identify patient and family needs and unmet needs, strengths and assets. Assess biopsychosocial needs of at-risk patients, i.e., single parents, substance abuse, complex medical patients, etc. Build care relationships among family and team; support the primary caregiving role of the family. Promote teams and actively participate in daily huddles. As a member of the care team, develop and implement patient care plans with family/youth/team (emergency plan, medical summary and action plan as appropriate). Complete forms such as DFS, FMLA, SSI, etc., and write letters for housing, nursing care, medical necessity, etc.; research, find and link resources, services and supports with/for the patient/family. Arrange for supplies and equipment. Assist with getting insurance coverage for patients without insurance. Coordinate inter-organizationally among family, the medical home and involved agencies. Identify community resources and track select community and specialty referrals. Connect to and understand community resources, i.e., WIC, food stamps, DME providers, advocacy groups, schools, financial assistance, counseling, anger management classes, special needs camps or inner-city camps. Refer patients to early intervention and public health nurses, help office staff and parents navigate through the school system, and help with IEPs. Promote/document Quality Improvement Cycles. Responsible for generating required data. Facilitate the NCQA process at the office working in close collaboration with the medical home liaison. Non-Essential Functions Facilitate family access to medical home providers, staff and resources. Actively participate on office teams. Help educate patients on using medical services appropriately; improve patient/family health care literacy, especially for at-risk patients. Performance Skills Relevant experience, or the equivalent, in community-based pediatrics, home health care or primary care, particularly in the care and service of vulnerable populations such as children/youth with special health care needs (CYSHCN). Leadership, advocacy, communication, education, counseling and resource research skills. Core philosophy or values consistent with a family-centered approach to care. Culturally effective capabilities demonstrating a sensitivity and responsiveness to varying cultural characteristics and beliefs. Experience with continuous quality improvement intiatives. The Registered Nurse provides nursing care to patients in the primary care pediatrics practices as directed by providers. The position is responsible for patient flow and assisting providers as necessary with examination, procedures and administration of immunizations that are adapted to the patient's development stage. Performs patient assessments. Performs and documents patient intake (height, weight, head circumference, blood pressure, vision and hearing screening, reason for visit, and chief complaint). Assists providers with procedures, including, but not limited to, suturing, suture removal, sterile procedures, minor surgical procedures, gyn exams and catheterizations. Performs phlebotomy, via venipuncture or capillary techniques, to obtain blood specimens. Transcribes, collects, labels and arranges transport for lab specimens. Performs back office laboratory tests. Administers immunizations, oral and/or injectable medications. Administers PPD and interprets results. Orders and stocks appropriate inventory of medical-surgical supplies for lab and exam rooms. . Triages patient calls utilizing delineated protocols under the supervision of the provider. Provides age- and developmentally appropriate care to infants, children, adolescents and adults. Monitors patient flow, including preparing patients and rooms for physicals and sick visits. Work hours: Monday-Friday, hours may vary from 8:00am to 8:00pm. Must be flexible with work hours. Requirements Pediatric experience preferred. Bilingual candidates preferred. Must have an active DE Nursing license. Outpatient experience preferred. BSN preferred. CPR certification with the American Heart Association required. Collaborates with Staff Development Clinical Instructors to provide educational resources for staff. Requirements BSN preferred. Minimum of three years experience as a Registered Nurse, pediatrics required. Experience with venous access, central venous catheters and invasive monitoring lines preferred. Current licensure with the Delaware Board of Nursing. Approaches patient care team interactions with fairness, consistency and objectivity, and supportive of a culture that enhances nursing performance, job satisfaction and patient care team retention. The Assistant Nurse Manager assists in systematically evaluating the quality and effectiveness of nursing practice: Participates on interdisciplinary teams to evaluate clinical practice. Utilizing the results of quality-of-care data, assists in implementing changes in clinical practice. Assists in the development and implementation of policies, procedures and practice guidelines to improve quality of care. Enures compliance with Hospital and departmental dashboard goals. Assists Nurse Manager in developing budget and budget compliance. Identifies and secures resources as necessary to maintain quality patient care. Completes payroll biweekly if deemed appropriate. Actively participates in departmental activities and plan for promoting patient/family and employee satisfaction. Monitors documentation of daily staffing; monitors documentation of admissions, discharges and transfers. Monitors staffing schedules. Administering immunizations (oral and/or injectable medications). Administering PPD and interpreting results. Performing phlebotomy via venipuncture or capillary techniques. Work hours: This position will require some evening hours, approximately until 7:00pm, 2-3 days per week. There is a weekend commitment for rotation in the coverage for the weekend primary care clinic, which is approximately 2 weekends per year.  This position may require some additional weekend hours in the future. Requirements BSN preferred. Primary care pediatric experience preferred. Must have or be eligible for a DE Nursing License. Must have a CPR certification with the American Heart Association. Primary Function This position is responsible for providing leadership in the Cardiac Intensive Care Unit and the 2B/Telemetry Unit to direct patient care in collaboration with the care team. Essential Functions Clinical Judgment Develops, implements, evaluates and modifies clinical practice to meet individual patient and family needs. Collects and interprets complex patient data.  Focuses on key elements of the situation while sorting out extraneous detail and exercises clinical judgment based on an immediate grasp of the whole picture for the patient population. Job Description: Telephone Triage Nurse, RN, Casual Midnights w/ Some Eves (BSN, Pediatrics) Nemours is seeking a Telephone Triage Nurse to join our team in Wilmington, DE. This position is casual, 32 hours in 6 weeks; emphasis will be on working midnights 12am-8:30am, both weekends and weekdays to fill department needs. Other shifts may include variable shifts weekday evenings, weekend day shift, evenings and overnights. While on orientation, will be required to work weekdays 4pm-12am and weekend day or evening shift for the first 90 days. Placing outbound follow-up calls to caregivers as needed. Must be able to work independently and make critical decisions autonomously as this is a complete work-at-home position after completion of orientation. Candidate should possess excellent communication and listening skills and the ability to multitask, make time-sensitive decisions and function well independently. Must be able to work independently from home in a private workspace. Current PA, DE and NJ nursing license or ability for license reciprocity in these states. Requirements RN required; BSN preferred. 3-5 years pediatric experience; nurse triage or primary care nursing experience preferred.
