Assists the department head in implementing, organizing and evaluating all services of the Home Care Program. Ensures services are rendered according to the plan of care and in accordance with policies and procedures within the Home Care Department. Supervises team to ensure work flows and processes are adhered to. Supports mission to provide the highest quality care by complying with regulatory guidelines, meeting facility standards and obtaining appropriate certification..   Essential Duties and Job Responsibilities include the following: Participates in the ongoing evaluation of all field clinicians within their designated team and their provision of care to ensure compliance with Home Care Program policies Conducts employee Performance Evaluations through daily performance, clinical record review and outcome of field evaluations Coordinates and integrates plans for continuity of patient care particularly with interdisciplinary team. Participates in system-wide quality improvement program. Supervises the implementation of departmental quality activities, as assigned. Assists in preparation for accreditation and program certification surveys. Involves staff at all levels in survey compliance. Recommends and implements systems for documentation necessary to maintain accountability and compliance with external regulations and internal standards, and standardization within SMHCS. Enforces the operations structure for the department and recommends work systems to ensure optimum utilization and coordination of personnel and resources. Makes home visits to assess, evaluate and provide direct therapy responsibilities, as indicated or needed. Provides feedback to staff when opportunities for improvement/excellence are observed. Performs coaching/disciplinary action as necessary, and with the approval of the department head. Participates in recruitment and retention efforts, as requested. Promotes customer friendly service. Investigates and resolves all customer complaints. Submits written reports within established time frames. Provides adequate opportunity for staff training and education. Coordinates staffing to meet in-service requirements. Assists in orientation of new staff. Monitors daily admissions, discharges, hospitalizations, incidents and complaints and reports outcomes appropriately as needed. Responsible to ensure that all clinical field staff are meeting their productivity standards and monitors their schedules to ensure compliance. Reviews and tracks timely submission of all necessary documentation, MD orders, and ensures SOC's and therapy evaluations are made according to department policy and regulation. Supervises all assigned clinical and office activities of Team. Communicates as appropriate with staff, physicians, referral sources and clients. Job Description: TITLE Nurse Practitioner/Physician Assistant - Nights  REQ NUMBER MED-17-00011   DESCRIPTION St. Mary's Hospital for Children, located in Bayside, is currently seeking a qualified Nurse Practitioner or Physician Assistant to join their team of professionals! This qualified team player will deliver quality primary and sub-acute care to pediatric patients ranging in age from newborn to adolescence. This includes ordering medications, therapies, tests, and other procedures according to, but not limited to, mutually agreed upon practice protocols which are determined in collaboration between Nurse Practitioner/Physician Assistant and attending Physician. Responsible for monitoring responses to interventions and coordinating care utilizing all members of the interdisciplinary team. Plays a central role in communicating with family members and outside agencies to provide comprehensive care and discharge planning. This is a full time position 6 shifts (8pm-8am) in a two week pay period. This position will also rotate weekend and holidays with team members. JOB DUTIES / RESPONSIBILITIES 1. Performs all aspects of resident care which includes ongoing physical exams; ordering of appropriate diagnostic studies, consultations by subspecialists, treatments and medications; reevaluating responses to treatments; and records all encounters and pertinent clinical information in the Electronic Medical Record as progress notes or Medical Monthly Progress Notes. All responsibilities are in accordance with scope of practice and in collaboration with the supervising physicians. 2. Upon admission, performs and documents a complete history and physical assessment for assigned residents within 24hours and have reviewed and cosigned by physician. Initiates a realistic and appropriate treatment plan and coordinates care accordingly. 3. Communicates with staff/families regarding changes in resident condition and with other providers via accurate and timely signout. 4. Prescribes immunizations, as needed, according to the American Academy of Pediatrics guidelines. Monitors immunizations records of all inpatients to be in compliance with AAP and NYS guidelines. Notifies families according to hospital policies of upcoming immunizations. 5. Serves as a clinical resource for nursing staff/resident/family. Aids in educations of fellows, residents, medical students, along with other medical providers. 6. Facilitates coordination of care between St. Mary's Hospital for Children, outside agencies, and tertiary care facilities. Documents all such communication in the resident medical record. 7. Participates in facility’s Quality Improvement Program as well as Performance Improvement Projects 8. Shares gaps in hospital coverage in house with other medical providers. Aids and assists with in house coverage during disasters/emergencies/storms/PTO schedules. 9. Participates in departmental meetings, rounds, lectures, and educational activities in person or via conference call when possible. 10. Completes all mandated hospital compliance training in timely fashion. 11. Follows all required infection control, safety, and confidentiality guidelines/mandates. 12. Trained to complete the death certificate utilizing the NYC Electronic Death Registry System (EDRS) and completes such certificate in a timely fashion in event of a death. 13. Takes on clinical tasks as deemed appropriate by Medical Director. Quality improvement nurses are responsible for optimizing nursing care in a facility. Depending on a healthcare facility’s strengths and opportunities, its quality improvement nurse jobs  can include varying duties. Some of the tasks could include recommending new technology, developing a better system for cataloging healthcare information, or creating pamphlets for frequently asked questions. There is no “typical” day in the lives of quality improvement nurses.

QI nurse jobs require problem-solvers who are extremely knowledgeable about the nursing field. They have advanced nursing degrees and generally have experience in the field before becoming a quality improvement nurse. This is a managerial position with much of the work done behind-the-scenes.
