Job Description: Primary Clinical Services Coordinator (Bilingual Family Nurse Practitioner) Apply practiced nursing knowledge in a distinctive medical facility. Provide expert nursing care in a Physician Practice area as a Clinical Services Coordinator. The Clinical Services Coordinator utilizes knowledge of nursing processes to plan, implement, evaluate, and document care. Job Description: Nurse Case Manager - BX, MH, SI and QNS   A.           POSITION SUMMARY:   Coordinates the delivery of health care services for persons receiving services in an assigned program unit. Makes nursing assessments and diagnoses.  Works in conjunction with other members of the Interdisciplinary Team (IDT) to achieve positive health outcomes for persons receiving services.    B.            POSITION DUTIES:   1.     Coordinates the delivery of routine and specialty health care services to address assessed                                                      needs of persons receiving services in assigned program unit.    Assesses and establishes health care outcomes for persons receiving services.  Updates assessments on at least a semiannual basis or as indicated. Maintains health care profiles.   3.     Acts as a liaison to communicate health information between and among team members, families, other agencies, hospitals, clinics and all other service providers.    4.     Completes all required written documentation.  Charts health care progress as indicated by acute illnesses and/or complex ongoing medical problems.    5.      Evaluates the effectiveness of preventive/ongoing health care and makes recommendations/ referrals for alternative care, as necessary.    6.   Conducts general and individual specific health related trainings for persons receiving services, direct care staff, and other team members as needed.       7.   Assigns Direct Care staff to perform health care treatments/procedures after having taken into consideration the complexity of the procedure(s), the skill/experience of the staff and the health condition(s)/status of the person receiving services.  Initially trains the staff in the task/activity and regularly observes/monitors to ensure all treatments/procedures are performed correctly.     8.     In conjunction with the Residential Coordinator, supervises AMAP staff and works with the Nursing Educator to ensure all AMAP staff are initially certified and annually re-certified.     9.     Provides on call coverage via cell phone between the hours of 6:00 am – 6:00 pm, Monday thru Friday.  Is prepared to go onsite if an on call problem warrants a nurse’s visit.    10.   Maintains all medical standards as set forth in agency policy and OPWDD regulations.     11. Job Description: Overnight Registered Nurse – SI (11pm-7am)   A.           POSITION SUMMARY:   Directly coordinates and delivers health care for persons receiving services in assigned site to achieve positive health outcomes. Responsible for nursing assessment, writing of diagnoses, developing and intervening in the persons receiving services health care plans to ensure quality care in the most efficient manner. Advocates for health and accesses hospital and specialty clinic services. Provides supervision to the Licensed Practical Nurse(s) on shift.   B.            POSITION DUTIES:   Assesses each person receiving services to determine and identify needs, issues, resources, and health care outcomes. Identifies individual’s development strengths and health care needs.  Obtains emergency medical care when necessary. Develops and updates health care plan to address assessd needs. Provides direct medical/nursing services to assigned individuals as presented. Oversees the implementation of the health care plan by training persons receiving services, family, and/or caregivers. Trains and oversees staff in carrying out treatment plans relating to health needs on an ongoing basis. Evaluates effectiveness of plan in meeting outcomes. Communicates outcomes to patients, families, caregivers, or service providers. Refers to other health providers, as needed. Schedules follow-up medical appointments according to medical recommendations. Ensures proper documentation of persons receiving services care and progress. Details acute and chronic health problems and interventions; reviews, medication, hospitalizations, clinics, weights, and dietary concerns. Identifies the need for, and arranges and conducts conferences to facilitate resolution of complex issues. Consults with Charge Nurse and/or Nursing Supervisor, as needed. Maintains ongoing communication with attending physicians, supervisors, and other health care practitioners (Clinicians). Works collaboratively with all involved personnel in managing the person receiving service’s care from admission through discharge. May accompany persons receiving services to clinics, recreation outings, social outings, etc. Develops strategies to prevent acute hospital admissions and/or deteriorations. Reviews incoming clinic visit summaries and chart in progress notes the results of doctor visits then implements orders in a timely manner. Develops and updates patient’s profile with the Residential Coordinator or other service providers for use at clinic appointments and hospitals, to include presenting problems and current medication list. Conducts on-going reviews of medication administration documentation and monthly reviews of medexes. Conducts regularly scheduled observations and pouring (procedures) of medication administration procedures performed by medication certified personnel. Regularly reviews procedures for medication receipt storage or accountability with Charge Nurse providing assistance in implementing corrective action as necessary. Provides direct oversight to Licensed Practical Nurse on shift, reporting problems and corrective action to Charge Nurse. In the absence of the Residential Coordinator and/or Shift Supervisor, directly supervises Direct Care staff for all individual care responsibilities. Maintains close contact with individual’s Day Program Nurse to ensure all medical/nursing needs are met in the Day Program. Provides health assessments and doctor’s orders where needed. Performs all other duties as assigned.   C.            REQUIREMENTS:   NYS Licensed Registered Nurse (BSN minimum). Minimum 2 years experience working with persons with disabilities. Extensive knowledge of pertinent health resources. Excellent oral and written communication skills. CPR Certification. Assumes accountability for dissemination of information to ANA stakeholders (internal and external).   QUALIFICATIONS REQUIRED   Education: Bachelor’s degree required.  Preference for RN, APRN or JD     Related Work Experience: Ten or more years of demonstrated, high-level analysis, research, grant activity, publication, teaching, and/or practice in areas relevant to the position as demonstrated by a substantial record in several of the following areas: Publication work should include a significant, ongoing record of individual and co-authored work published in various outlets, including policy papers, research summaries, payment reviews, journals, chapters in books, monographs and book reviews.   Association experience is highly desirable. Prior work with nurses or nursing groups on professional nursing issues strongly preferred. Evidence of collaboration with other health care disciplines strongly preferred.   Skills: Ability to rapidly analyze and explain complex nursing, and healthcare, and legal concepts in a clear and concise manner. Must be able to formulate, evaluate and explain related policies. Evidence of effective public speaking at the state, regional, national or international level. Proficiency in working with individuals with various backgrounds and levels of sophistication on nursing practice and health policy, including ANA staff, elected and appointed officials and other organizations. Significant record of individual and/or co-authored publications in nursing and/or healthcare related journals. Demonstrated ability to effectively lead projects and workgroups within and outside of the organization. Strong proficiency with computer software including Microsoft software applications and other related software.
