Job Description: Primary Clinical Services Coordinator (Bilingual Family Nurse Practitioner) Apply practiced nursing knowledge in a distinctive medical facility. Provide expert nursing care in a Physician Practice area as a Clinical Services Coordinator. The Clinical Services Coordinator utilizes knowledge of nursing processes to plan, implement, evaluate, and document care. TBHC has been a leader in the provision of complete, efficient, and community-responsive care since 1839, and is a Top Performer for Key Quality today.  We offer an extensive network of primary and specialty services that improve outcomes for our diverse constituents. Both hands-on experience and administrative acumen is needed to succeed in this role. The chosen professional will provide direct primary care to all patients in the assigned ambulatory care unit, including obtaining histories, performing diagnostic and therapeutic procedures, and documenting all encounters in the patient’s medical record.The Clinical Services Coordinatorprovides leadership to new programmatic initiatives addressing the clinical and psychosocial needs, while displaying professional practice as set forth with standard of care, and the institution’s mission and goals.   Other responsibilities include: Reviewing and charting patient’s ongoing medications and alerting providers of discrepancies Performing venipuncture, starting IV lines, accessing central catheters Drawing and labeling bloods, urine, CSF and sends to appropriate laboratories Arranging admissions and prior authorizations Conducting age-appropriate education, treatment education, and adherence monitoring and prevention services to all patients Conducting Performance Improvement (PI) activities   Qualified candidate must: Have Masters Degree in Nursing or completion of Advance Nursing Practice Program Possess experience as Family Nurse Practitioner Be a New York State Licensed Registered Nurse and Nurse Practitioner Have a strong understanding of business  fundamentals and hospital operations Have Basic Cardiac Life Support (BCLS) Certification Have strong communications, business administration, and human resources skills Be fluent in both English and Spanish or English and Creole   Team members here enjoy a stimulating work setting, a location in the heart of vibrant and historic downtown Brooklyn, and a complete salary/benefits package.
