Job Description: Nurse Case Manager - BX, MH, SI and QNS   A.           POSITION SUMMARY:   Coordinates the delivery of health care services for persons receiving services in an assigned program unit. Makes nursing assessments and diagnoses.  Works in conjunction with other members of the Interdisciplinary Team (IDT) to achieve positive health outcomes for persons receiving services.    B.            POSITION DUTIES:   1.     Coordinates the delivery of routine and specialty health care services to address assessed                                                      needs of persons receiving services in assigned program unit.    Assesses and establishes health care outcomes for persons receiving services.  Updates assessments on at least a semiannual basis or as indicated. Maintains health care profiles.   3.     Acts as a liaison to communicate health information between and among team members, families, other agencies, hospitals, clinics and all other service providers.    4.     Completes all required written documentation.  Charts health care progress as indicated by acute illnesses and/or complex ongoing medical problems.    5.      Evaluates the effectiveness of preventive/ongoing health care and makes recommendations/ referrals for alternative care, as necessary.    6.   Conducts general and individual specific health related trainings for persons receiving services, direct care staff, and other team members as needed.       7.   Assigns Direct Care staff to perform health care treatments/procedures after having taken into consideration the complexity of the procedure(s), the skill/experience of the staff and the health condition(s)/status of the person receiving services.  Initially trains the staff in the task/activity and regularly observes/monitors to ensure all treatments/procedures are performed correctly.     8.     In conjunction with the Residential Coordinator, supervises AMAP staff and works with the Nursing Educator to ensure all AMAP staff are initially certified and annually re-certified.     9.     Provides on call coverage via cell phone between the hours of 6:00 am – 6:00 pm, Monday thru Friday.  Is prepared to go onsite if an on call problem warrants a nurse’s visit.    10.   Maintains all medical standards as set forth in agency policy and OPWDD regulations.     11. Participates in meetings as indicated including but not limited to IDT, Day Program, Human Rights, Departmental, Borough and specially arranged meetings.     12.  Performs all other duties as assigned or indicated including but not limited to: completing surrogate packets, writing incident reports, assisting in developing policies, procedures and forms; and establishing working systems.    C.  REQUIREMENTS:   1.    NYS Registered Nurse.  Bachelors Degree preferred. 2.    Prior nursing experience and/or experience working with persons with developmental disabilities preferred.  3.    Excellent oral and written communication skills.  4.    Ability to travel to and access multiple locations throughout the metropolitan area.  5.    Able to work flexible hours and/or days. 6.    Performs all job responsibilities in full compliance with all applicable laws, rules, regulations, policies and procedures.  Adheres to the agency’s corporate compliance program, including the code of business conduct, the NYS and federal false claims act, whistleblower protection and mandatory reporting.  Completes corporate compliance training at least annually.  Fully cooperates in all corporate compliance investigations and reviews.    D.      SUPERVISES:     Medical Case Manager Counselors   Apply by sending your resume via e-mail to jcooke@cpofnys.org or fax to: 212-290-8475, “Attention: Jamilah Cooke”.

Job Description: POSITION:  Senior Policy Advisor COMPANY:  American Nurses Association LOCATION:  Silver Spring, MD TO APPLY:  Apply online or email resume to chet.reisler@ana.org     The focus of the Senior Policy Advisor role is to provide leadership between and among departments in the review of research, analysis of trends and issues, provision of consultative services to C/SNAs, OAs and ANA Enterprise staff, development of policy and practice position statements and white papers, and to disseminate meaningful information to ANA Enterprise Staff, and the broader nursing community, through effective communication, publication and presentations.     DUTIES: Analyzes legislation at the state and federal level for its impact on RNs/APRNS.  Analyst expected to have knowledge of reimbursement systems and impact on payment policy and coverage of specific populations. Analyzes proposed and final Federal agency rules and regulations (published in the Federal Register) that impact health care and the nursing profession. Assumes accountability for dissemination of information on proposed comment opportunities to ANA stakeholders (internal and external). Coordination on regulatory comment letters developed for submission by ANA and assumes accountability for planning, direction, management and analysis of related activities. Analyzes published research for purposes of integration and translation into practice and/or policy. Assumes accountability for the planning, direction, management and analysis of related activities. Monitors current significant trends, issues and strategies related to assigned or related projects, policies, positions, issues and areas of expertise. Monitors and evaluates developments significant to health care policy and nursing practice. Communicates knowledge through well-written papers, reports, articles, presentations and other communication venues to inform the work of the department, the association and the wider nursing community. Provides and coordinates health policy and nursing practice consultative services for C/SNAs, Organizational Affiliates and ANA Enterprise staff, working in and leading cross collaborative teams within the Entity. Writes and publishes materials for journals and other print materials including ANA publications. Identifies opportunities, develops and submits abstracts for poster and podium presentations.  As requested, conducts news interviews and provides content to ANA leadership. Identifies significant nomination opportunities published in the Federal Register. Assumes accountability for dissemination of information to ANA stakeholders (internal and external).   QUALIFICATIONS REQUIRED   Education: Bachelor’s degree required.  Preference for RN, APRN or JD     Related Work Experience: Ten or more years of demonstrated, high-level analysis, research, grant activity, publication, teaching, and/or practice in areas relevant to the position as demonstrated by a substantial record in several of the following areas: Publication work should include a significant, ongoing record of individual and co-authored work published in various outlets, including policy papers, research summaries, payment reviews, journals, chapters in books, monographs and book reviews.   Association experience is highly desirable. Prior work with nurses or nursing groups on professional nursing issues strongly preferred. Evidence of collaboration with other health care disciplines strongly preferred.   Skills: Ability to rapidly analyze and explain complex nursing, and healthcare, and legal concepts in a clear and concise manner. Must be able to formulate, evaluate and explain related policies. Evidence of effective public speaking at the state, regional, national or international level. Proficiency in working with individuals with various backgrounds and levels of sophistication on nursing practice and health policy, including ANA staff, elected and appointed officials and other organizations. Significant record of individual and/or co-authored publications in nursing and/or healthcare related journals. Demonstrated ability to effectively lead projects and workgroups within and outside of the organization. Strong proficiency with computer software including Microsoft software applications and other related software. Demonstrated ability to independently prioritize and manage multiple high-level complex projects and complete within specified timeframes. Demonstrated ability to work effectively and collaboratively with staff at all levels and across organizational units. Must understand and be able to provide guidance, advice and technical assistance for the review and formulation of federal legislative and regulatory proposals.

Health unit coordinators help maintain healthcare facilities by assisting nurses and other staff members in completing non-clinical tasks.

Often acting as a liaison between patients and medical staff, health unit coordinators may be responsible for a variety of tasks. Although responsibilities vary depending on the facility, typical duties include setting up admittance charts, data entry, ordering lab tests, maintaining patient records and managing facility communications.

Health unit coordinator jobs can be found in a variety of medical facilities, including hospitals, clinics, private practices and more.  Although these positions are usually clerical, they often involve patient interaction, so coordinators must have strong verbal and written communication skills.

Currently, many facilities provide on the job training. However, candidates are increasingly required to have certification from the National Association of Health Unit Coordinators (NAHUC).
