Charge Nurse - SI A. POSITION SUMMARY: Directly oversees the provision of services provided to patients by clinic physicians, clinicians, RNs, LPNs and DCCs to ensure that all health needs are addressed in an appropriate and efficient manner. Provides direct oversight and complete responsibility for the nursing assessment and initiating health care plans to ensure that the highest quality care is being rendered in the most efficient manner to the individuals served. Oversees the accurate implementation of the health care plan by the nursing staff. Trains and oversees staff in carrying out treatment plans relating to the health needs of the individuals served on an ongoing basis. Evaluates effectiveness of care plans in meeting all outcomes, goals and objectives to ensure that the best care is provided. Communicates outcomes to individuals, families, caregivers and or service providers. Provides updated reports and ensures proper documentation by staff are being delivered in a professional and timely manner. B. POSITION DUTIES: 1. Supervises RNs and provides oversight and instructions to the nursing staff and direct care staff. 2. In collaboration with Clinical Nursing Supervisor, establishes systems for day-to-day medical/nursing operations. 3. Monitors compliance with established policies and procedures and takes corrective action to maintain high standards of nursing care. 4. Monitors activities in all clinical areas to ensure that quality services are provided within the framework of Agency policies and procedures and all applicable government regulations. 5. Completes monthly schedules for nurses. 6. Ensures that the facility is well stocked with medical supplies and that all equipment is in working order. Maintains appropriate logs and control count sheets. 7. Collaborates with the physician on the disposition of unscheduled/priority appointment requests. 8. Conducts monthly nurses meetings and provides verbal and written guidance as needed. Oversees and identifies the need for and arranges and conducts conferences to facilitate resolution of complex issues. 9. Provides in-service training for 24 hour nurses as required. Develops and updates patient’s profile with the Residential Coordinator or other service providers for use at clinic appointments and hospitals, to include presenting problems and current medication list. Assigns nursing staff to perform health care treatments/procedures and having taken into consideration the complexity of the procedure(s), the skill/experience of the staff and the health conditions/status of the person receiving services. Initially trains the staff in the task/activity and regularly observes/monitors to ensure all treatments/procedures are performed correctly. 10. Oversees prior authorizations for medications, feedings and transportation. 11. Supervises the preparation packages for the Surrogate Court and CAB. Attends the surrogate court proceedings with or without the day shift primary nurse. 12. Attends all annual and semi-annual ISP meetings/conferences in the various day programs attended by the individuals we serve in the ICF 14. 13. In the absence of the Clinical Nursing Supervisor, the Charge Nurse is responsible for overall ICF operations and supervision of al nursing staff. 14. Maintains a safe, therapeutic ICF environment. 15. Performs all other duties as assigned. C. REQUIREMENTS: 1. NYS licenses Registered Nurse. Minimum three years nursing experience with one year in supervision. 3. C.P.R. certification. 4. Extensive knowledge of pertinent health resources. 5. Excellent oral and written communication skills. 6. Able to work flexible hours and/or days. 7. Nurse Case Manager - BX, MH, SI and QNS A. POSITION SUMMARY: Coordinates the delivery of health care services for persons receiving services in an assigned program unit. Makes nursing assessments and diagnoses. Works in conjunction with other members of the Interdisciplinary Team (IDT) to achieve positive health outcomes for persons receiving services. B. POSITION DUTIES: 1. Coordinates the delivery of routine and specialty health care services to address assessed needs of persons receiving services in assigned program unit. Assesses and establishes health care outcomes for persons receiving services. Updates assessments on at least a semiannual basis or as indicated. Maintains health care profiles. 3. Acts as a liaison to communicate health information between and among team members, families, other agencies, hospitals, clinics and all other service providers. 4. Completes all required written documentation. Charts health care progress as indicated by acute illnesses and/or complex ongoing medical problems. 5. Evaluates the effectiveness of preventive/ongoing health care and makes recommendations/ referrals for alternative care, as necessary. 6. Conducts general and individual specific health related trainings for persons receiving services, direct care staff, and other team members as needed. 7. Assigns Direct Care staff to perform health care treatments/procedures after having taken into consideration the complexity of the procedure(s), the skill/experience of the staff and the health condition(s)/status of the person receiving services. Initially trains the staff in the task/activity and regularly observes/monitors to ensure all treatments/procedures are performed correctly. 8. In conjunction with the Residential Coordinator, supervises AMAP staff and works with the Nursing Educator to ensure all AMAP staff are initially certified and annually re-certified. 9. Provides on call coverage via cell phone between the hours of 6:00 am – 6:00 pm, Monday thru Friday. Is prepared to go onsite if an on call problem warrants a nurse’s visit. 10. Maintains all medical standards as set forth in agency policy and OPWDD regulations. 11. Participates in meetings as indicated including but not limited to IDT, Day Program, Human Rights, Departmental, Borough and specially arranged meetings. 12. Performs all other duties as assigned or indicated including but not limited to: completing surrogate packets, writing incident reports, assisting in developing policies, procedures and forms; and establishing working systems. C. REQUIREMENTS: 1. NYS Registered Nurse. Bachelors Degree preferred. 2. Prior nursing experience and/or experience working with persons with developmental disabilities preferred. 3. Excellent oral and written communication skills. 4. Ability to travel to and access multiple locations throughout the metropolitan area. 5. Able to work flexible hours and/or days. 6.
