Job Description: Charge Nurse - SI   A.           POSITION SUMMARY:   Directly oversees the provision of services provided to patients by clinic physicians, clinicians, RNs, LPNs and DCCs to ensure that all health needs are addressed in an appropriate and efficient manner.  Provides direct oversight and complete responsibility for the nursing assessment and initiating health care plans to ensure that the highest quality care is being rendered in the most efficient manner to the individuals served.  Oversees the accurate implementation of the health care plan by the nursing staff.  Trains and oversees staff in carrying out treatment plans relating to the health needs of the individuals served on an ongoing basis.  Evaluates effectiveness of care plans in meeting all outcomes, goals and objectives to ensure that the best care is provided.  Communicates outcomes to individuals, families, caregivers and or service providers.  Provides updated reports and ensures proper documentation by staff are being delivered in a professional and timely manner.      B.            POSITION DUTIES:   1.     Supervises RNs and provides oversight and instructions to the nursing staff and direct care staff.      2.     In collaboration with Clinical Nursing Supervisor, establishes systems for day-to-day medical/nursing operations.   3.     Monitors compliance with established policies and procedures and takes corrective action to maintain high standards of nursing care.    4.     Monitors activities in all clinical areas to ensure that quality services are provided within the framework of Agency policies and procedures and all applicable government regulations.   5.   Completes monthly schedules for nurses.    6.    Ensures that the facility is well stocked with medical supplies and that all equipment is in working order.  Maintains appropriate logs and control count sheets.    7.     Collaborates with the physician on the disposition of unscheduled/priority appointment requests.    8.     Conducts monthly nurses meetings and provides verbal and written guidance as needed.  Oversees and identifies the need for and arranges and conducts conferences to facilitate resolution of complex issues.   9.      Provides in-service training for 24 hour nurses as required.  Develops and updates patient’s profile with the Residential Coordinator or other service providers for use at clinic appointments and hospitals, to include presenting problems and current medication list.  Assigns nursing staff to perform health care treatments/procedures and having taken into consideration the complexity of the procedure(s), the skill/experience of the staff and the health conditions/status of the person receiving services.  Initially trains the staff in the task/activity and regularly observes/monitors to ensure all treatments/procedures are performed correctly.   10. Oversees prior authorizations for medications, feedings and transportation.   11.  Supervises the preparation packages for the Surrogate Court and CAB.  Attends the surrogate court proceedings with or without the day shift primary nurse.   12.   Attends all annual and semi-annual ISP meetings/conferences in the various day programs attended by the individuals we serve in the ICF 14.    13.   In the absence of the Clinical Nursing Supervisor, the Charge Nurse is responsible for overall ICF operations and supervision of al nursing staff.   14.   Maintains a safe, therapeutic ICF environment.   15.   Performs all other duties as assigned.        C.  REQUIREMENTS:   1.    NYS licenses Registered Nurse. Job Description: Overnight Registered Nurse – SI (11pm-7am)   A.           POSITION SUMMARY:   Directly coordinates and delivers health care for persons receiving services in assigned site to achieve positive health outcomes. Responsible for nursing assessment, writing of diagnoses, developing and intervening in the persons receiving services health care plans to ensure quality care in the most efficient manner. Advocates for health and accesses hospital and specialty clinic services. Provides supervision to the Licensed Practical Nurse(s) on shift.   B.            POSITION DUTIES:   Assesses each person receiving services to determine and identify needs, issues, resources, and health care outcomes. Identifies individual’s development strengths and health care needs.  Obtains emergency medical care when necessary. Develops and updates health care plan to address assessd needs. Provides direct medical/nursing services to assigned individuals as presented. Oversees the implementation of the health care plan by training persons receiving services, family, and/or caregivers. Trains and oversees staff in carrying out treatment plans relating to health needs on an ongoing basis. Evaluates effectiveness of plan in meeting outcomes. Communicates outcomes to patients, families, caregivers, or service providers. Refers to other health providers, as needed. Schedules follow-up medical appointments according to medical recommendations. Ensures proper documentation of persons receiving services care and progress. Details acute and chronic health problems and interventions; reviews, medication, hospitalizations, clinics, weights, and dietary concerns. Identifies the need for, and arranges and conducts conferences to facilitate resolution of complex issues. Consults with Charge Nurse and/or Nursing Supervisor, as needed. Maintains ongoing communication with attending physicians, supervisors, and other health care practitioners (Clinicians). Works collaboratively with all involved personnel in managing the person receiving service’s care from admission through discharge. May accompany persons receiving services to clinics, recreation outings, social outings, etc. Develops strategies to prevent acute hospital admissions and/or deteriorations. Reviews incoming clinic visit summaries and chart in progress notes the results of doctor visits then implements orders in a timely manner. Develops and updates patient’s profile with the Residential Coordinator or other service providers for use at clinic appointments and hospitals, to include presenting problems and current medication list. Conducts on-going reviews of medication administration documentation and monthly reviews of medexes. Conducts regularly scheduled observations and pouring (procedures) of medication administration procedures performed by medication certified personnel. Regularly reviews procedures for medication receipt storage or accountability with Charge Nurse providing assistance in implementing corrective action as necessary. Provides direct oversight to Licensed Practical Nurse on shift, reporting problems and corrective action to Charge Nurse. In the absence of the Residential Coordinator and/or Shift Supervisor, directly supervises Direct Care staff for all individual care responsibilities. Maintains close contact with individual’s Day Program Nurse to ensure all medical/nursing needs are met in the Day Program. Provides health assessments and doctor’s orders where needed. Performs all other duties as assigned.   C.            REQUIREMENTS:   NYS Licensed Registered Nurse (BSN minimum). Minimum 2 years experience working with persons with disabilities. Extensive knowledge of pertinent health resources. Excellent oral and written communication skills. CPR Certification.
