A healthcare organization on Long Island is actively seeking a Spanish-speaking Licensed Clinical Social Worker (LCSW) for a promising position with their growing medical staff. In this role, the Bilingual Licensed Clinical Social Worker (LCSW) will be responsible for providing crisis intervention, bio-psychosocial assessments, individual, family and group counseling and/or case management services to HIV positive patients, their partners, caregivers and family members, as well as other patients with issues related to chronic illness, aging, maternity, domestic violence or other social problems. Job Description

The Bilingual Licensed Clinical Social Worker (LCSW) will be responsible for

  • Providing mental health services to patients with behavioral health conditions
  • Development and formulation of treatment plans for patients in care work with psychiatrists on monthly basis
  • Handling active caseload of at least twenty-five patients
  • Supporting transition and coordination for chronically and persistently mentally ill to appropriate, alternate care
  • Documenting all patient encounters appropriately and maintain comprehensive case notes
  • Reviewing quality reports for site on all indicators monthly
  • Completing monthly program reports
  • Working cooperatively with entire HIV, medical and Behavioral Health team
  • Participating in and facilitating case conferences as well as other agency, school and/or community based meetings or trainings
  • Providing creative prevention services to individuals at risk of HIV infection and other chronic medical conditions or negative health or social factors
  • Providing pre and post test counseling; individual, group, family, marital and crisis intervention services
Required Skills
  • 1+ year of related Social Work experience
  • Master's Degree in Social Work
  • NYS Licensed Clinical Social Worker (LCSW)
  • Bilingual (English and Spanish)
  • Counseling and Case Management background
  • Previous experience with with the Behavioral and/or Psychiatric population 
  • Solid assessment and documentation skills
  • Patient oriented
  • Great interpersonal skills
  • Excellent communication skills (written and verbal)
  • Strong attention to detail
  • Highly organized
